• Voeren van keukentafelgesprekken en opstellen van integrale leef-zorgplannen volgens de 1-gezin-methode.
• Afhandelen van Veilig-Thuismeldingen en daadkrachtig borgen van de veiligheidsplannen.
• Draaien van bereikbare diensten voor crisissituaties in samenwerking met de crisisdienst.
• Verbinden van onderwijs, huisartsen en specialistische partners voor snelle zorg op maat.
• Coachen van teamleden en borgen van een kwalitatieve uitvoering van de Jeugdwet.

Functie-eisen

• Je hebt een afgeronde hbo-opleiding in Jeugdzorg, Maatschappelijk Werk of Welzijn.
• Je beschikt over een geldige SKJ-registratie.
